Social Services
Our social services are delivered by a dedicated team of Social Service Case Managers—all of whom are trained in delivering culturally-sensitive, empathetic, and humanistic care. Your Case Manager may be a social worker, counselor, case worker, or community health worker based on your unique needs.
We currently offer services through phone and in-person.
Social Services are offered in the following areas:
Health Access & Coverage
We review free and low-cost healthcare options near you, provide insurance plan counseling, and assist with any health insurance issues, including Medicaid appeals.
Prescription Medication Assistance
We help find discounts for your prescription drugs so you can stick with your treatment plan.
Prescription Corrective Lenses
We help community members receive free prescription corrective lenses for uninsured immigrants.
Immigration & Citizenship
We provide counseling on immigration status and how it relates to specific health access options.
Identification Card Assistance
Need government-issued identification? We help with your IDNYC application, New York City's municipal ID card.
Governmental Assistance
We review and research eligibility for government benefit programs, such as welfare and social security, assist with your application, and follow up as needed.
Social & Environmental Conditions
We provide support for social circumstances, including housing, transportation, family planning, safety, domestic violence and abuse, and more.
Preparing for your Social Services appointment
Please arrive 10 minutes prior to your appointment time with all the forms and information requests if we have sent them to you.
Remember to bring all relevant and necessary documentation related to your case. In most cases, we will let you know what to bring, but if you have any doubts, please bring the below documentation with you.
Common documentation includes:
Identification Card (government-issued ID such as Driver's License or IDNYC)
Proof of Income or Proof of Support from Family Member
Proof of Address such as Utility Bills for the past six months
Household Size and Total Household Income
During your appointment:
When you come in, you will be greeted in your language of choice by your Community Health Partner who will show you around, tell you what forms to fill out (and assist with the paperwork if needed), and accompany you throughout your appointment with your clinical partner.
